Birth Details (for name blocks)

If you wish to add your child's birth details please do so here.

Please fill in the details as stated below in the "Special Requirement" field. If you're requiring birth details for more than one set of blocks please add required details as "Block set 1", "Block set 2", and "Block set 3".

Eg: Block set one... 

1. Name:
2. Date
3. Time
4. Weight

Then adjust the "Quantity" field for as many names (block sets) which are required, for example, Tom, Harry, and Anna then only 3 is needed in the Quantity field
